Do You Accept My Insurance?

At our methadone clinics, we accept many insurance plans, including Medicare and Illinois Medicaid. Coverage applies across all locations unless noted.

Do You Accept Medicare?

Yes. Medicare is accepted across all states. Medicare plans typically cover methadone treatment and related services.

Do You Accept Medicaid?

Yes, Illinois Medicaid only.

Illinois Medicaid covers methadone treatment at all Revive locations in Illinois. There are no state-specific restrictions and no prior authorization requirements.

Yes. For patients without insurance, self-pay is only $50 per week.*

Appendix A — Financial Hardship Discount Policy (Excerpt)

Revive Treatment Centers may, after individualized review, reduce or waive copays/coinsurance and patient balances when documented financial hardship would otherwise create a barrier to medically necessary care. The program maintains a single, published fee schedule; collects income verification at intake; informs patients of third‑party benefits and costs not covered; and obtains a signed fee acknowledgment. Discounts are applied according to a sliding scale based on household size and income relative to the Federal Poverty Level (FPL).

Discounts are not offered in a manner that would violate payer contracts or law. The organization does not advertise free care, does not provide discounts in exchange for referrals or volume, and documents an individualized determination supported by objective evidence. Eligibility is time‑limited and reviewed at least annually or upon material change.
